Dr. Peter Schochet, Dr. Hauw Lie and their staff will provide you with patient information forms to assist you with your child’s healthcare needs. If you would like to make an appointment with us, please submit an online appointment request or call our Plano office at 972 981 3251.

Appointments

To make an appointment with Dr. Schochet or Dr. Lie, call us at 972-981-3251 between 8:30 AM to 5:00 PM, Monday thru Friday. Please have your insurance information and referral (if required by your insurance plan) available.

We appreciate that our patients’ lives are hectic and that no one likes to be kept waiting. That is why we try to keep our appointments in a timely manner. In the event of an emergency, however, we will make every effort to notify our patients of scheduling changes.

What to Bring to your Appointment

  1. Medical insurance cards
  2. Driver’s license/photo ID of responsible party
  3. New patient paperwork
  4. All medications in original bottles with labels (and spacers if used)
  5. Medical records, labs, x ray reports (or have them faxed prior to the appointment)
  6. A parent or legal guardian must accompany new patients under the age of 18. Signatures regarding medical decision-making must be submitted during the first visit.

Follow-Up Visits

Note:  On follow-up visits when only minor medical decision-making is required, grandparents, nurses’ aides, or other adults with a working knowledge of the child’s medical history may accompany children. Our office requires a written directive on file for such visits. This directive should be filed with our office prior to the first appointment and should be updated with our office on return visit.

Insurance

Dr. Peter Schochet accepts most insurance plans. Please contact us at 972-981-3251 to verify your individual plan prior to your appointment. Benefits are verified prior to each visit.

We provide insurance billing. Co-pays and unmet deductibles are collected at the end of each visit. Please note that costs not covered by your insurance are your responsibility. Medical supplies used during diagnostic testing are sometimes not covered by a patient’s plan. These fees (usually under $10) are collected at the end of each visit. Plans, which do offer coverage for medical supplies often require a coinsurance percentage on those items, or the charges may apply to a supply deductible.

Referrals are the responsibility of the patient, and without the proper referral we MUST reschedule. If a referral was not obtained prior to an appointment, a cash payment will be accepted at the time of the appointment.

For patients not covered by insurance our Billing Office will be happy to establish payment arrangements and can accommodate most budgets.

Dr. Schochet and Dr. Lie participate on some of the Medicaid plans.
